How to create an interactive form interface with human avatars and giving customization features of avatars hair/eye color for visitors?

I want to build up a form interface in my wordpress blog, where will be 4 cartoon human avatars and want to give a feature to my visitors that they will be able to select the casts' hair/eye color, which will reflect on the avatar image. After that they will be able to send those customization data to me by using a submit button.

Now my question is how can I able to do this by using HTML5, CSS3 and Javascript or jQuery except flash?
